The following items and documents are required to register for membership discounts and services:
– Valid State Sales Tax Certificate
– Completed IRS form W-9
– Valid business license
– Completed Designer Program form.
A copy of your Tax Certificate must be presented at the time of purchase to qualify for Designer Program discounts. If a copy of the certificate is not available at the time of purchase, it may be submitted later, but MUST be received prior to delivery of the order. No product will be delivered until a valid state Tax Certificate is on file with Ultra Modern Pool & Patio. Customer information must be shared for follow up purposes, warranty, and scheduling delivery. 50% Deposits required on special orders.
